Shockwave Therapy for Runners: Reducing Pain and Supporting Recovery

Running involves thousands of steps during a single session, with each stride producing impact forces up to three times your body weight. Over time, this repeated loading can irritate soft tissues, especially if training volumes increase quickly, recovery is limited, or footwear and running surfaces are not ideal. Even experienced runners are not immune, with common injuries including heel pain, tendon irritation, and knee discomfort. 

What is Shockwave Therapy? 

Shockwave therapy uses acoustic energy waves delivered directly to injured tissues through a handheld device. These sound waves create controlled microtrauma within the affected area, stimulating increased blood circulation, accelerating cellular repair, and reducing sensitivity in pain receptors. Importantly, shockwave therapy is non-invasive, requires no injections or medication, and can be delivered within a short physiotherapy appointment. 

Conditions in Runners That Benefit from Shockwave 

Shockwave therapy is supported by research for several chronic musculoskeletal conditions. Within the running population, it is particularly effective for: 

  • Plantar fasciitis – sharp heel pain, often worse in the morning or after long runs 
  • Achilles tendinopathy – pain and stiffness at the back of the ankle 
  • Patella tendinopathy (runner’s knee) – pain at the front of the knee, especially when running downhill or climbing stairs 
  • Shin splints (medial tibial stress syndrome) – aching pain along the shin bone linked to repetitive impact 
  • Hamstring tendinopathy – deep pain near the sit bone, aggravated by running or sprinting 

These injuries are not only painful but can become long-term problems that stop runners from training consistently. Shockwave therapy can help break the cycle of ongoing pain, making it a valuable addition to rehabilitation. 

What to Expect During Treatment 

A shockwave therapy session is straightforward. After assessment, your physiotherapist will apply the device to the affected area for several minutes. The sensation is often described as a series of pulses that may feel uncomfortable, but it is rarely described as painful. Most runners tolerate it well. Treatment is delivered without the need for anaesthetic, and you can usually continue with normal daily activities straight after. 

A full course usually involves several sessions, often once per week, combined with physiotherapy-led exercises to restore strength, mobility, and running technique. This combined approach ensures not only symptom relief but also a reduced risk of the problem recurring.

Traditional management such as rest, stretching, or anti-inflammatory medication can provide temporary relief but may not resolve the underlying problem.
