Shockwave Therapy for Achilles Tendinopathy

Feb 2, 2026

The Achilles tendon is the strongest tendon in the body, connecting the calf muscles to the heel bone. Despite its strength, it is also one of the most frequently injured tendons. Achilles tendinopathy is a common overuse condition, often seen in runners, sportspeople, and those who spend long periods on their feet.

Typical symptoms include pain and stiffness in the back of the ankle, especially first thing in the morning or during activity. Swelling or thickening of the tendon may also be present. Left untreated, the condition can last for months and interfere with walking, work, and sport.

At Chapman Physiotherapy, we treat many patients with Achilles tendon pain. Our clinicians combine detailed assessment, diagnosis, and evidence-based treatment options to reduce pain and restore movement. One of the most effective therapies we now provide at our Doncaster clinic is shockwave therapy.

What is shockwave therapy?

Shockwave therapy is a non-invasive treatment that delivers acoustic energy into the affected tendon. The pulses of energy increase blood flow, stimulate cellular activity, and encourage the tendon to heal. It also helps to break down disorganised scar tissue and trigger the body’s natural repair processes.

The treatment is particularly effective for tendon problems that have not improved with rest, stretching, or strengthening exercises alone. For many patients, it offers a way forward when symptoms have been persistent for several months.

How does it help Achilles tendinopathy?

Research has shown that shockwave therapy can provide significant benefits for both mid-portion and insertional Achilles tendinopathy. These include:

  • Pain reduction – shockwave helps to desensitise pain nerve fibres in the tendon.
  • Improved tendon healing – stimulating circulation and cell activity supports tissue repair.
  • Faster return to activity – patients are often able to resume walking, sport, and training sooner.

Several clinical studies have found that combining shockwave therapy with an exercise programme improves outcomes more than exercise alone. Eccentric strengthening exercises remain an essential part of Achilles tendon rehabilitation, and shockwave acts as an additional stimulus to speed recovery.

What happens during treatment?

A typical course involves 3–6 weekly sessions. Each session lasts around 15–20 minutes. The physiotherapist applies the shockwave probe directly to the tendon while you are lying comfortably on the treatment couch.

Patients often describe the sensation as uncomfortable but tolerable, similar to a tapping or vibrating feeling. The intensity can be adjusted to suit your tolerance. After treatment, some temporary redness or soreness may occur, but this usually settles within 24 hours.

Most patients notice improvements within the first few sessions. Long-term benefits continue as the tendon remodels and strengthens with exercise.
